I give permission for my youth to participate in the Sherwood Park Moroni's Quest and its associated activities. I understand this event takes place in an outdoor setting and that participants and leaders will be camping with food, restroom facilities, and safe drinking water provided.

I authorize the supervising adult leaders to provide emergency care and to approve necessary medical treatment for my youth in the event of illness or injury.

I understand that participants are expected to follow Church standards, event safety rules, and leader direction, and to conduct themselves in a Christlike manner. Participation is a privilege and may be revoked if a participant’s behavior is unsafe or inappropriate.

Media Release - Part 1 - Photo / Video / Audio / Works

Outside of the release forms already provided, the Sherwood Park Stake of the Church of Jesus Christ of Latter-Day Saints requires consent to record, copy, film or photograph your child's name, image, work, or performance (hereinafter collectively referred to as "Works") and to distribute these Works for the purpose of publishing, posting on Moroni's Quest website, social media, newspapers or for broadcasting on television and radio as determined by the Stake members responsible for Media around Sherwood Park Stake's Moroni's Quest. I hereby waive any right to approve the use of these Works now or in the future, whether the use is known or unknown. I understand the Works may appear in electronic form on the Internet or in other publications outside of the Sherwood Park Stakes control . I agree that I will not hold Stake or Church responsible for such unauthorized productions.

I consent to the participant’s Works being shared with and published/posted/viewed by Sherwood Park Stake members assigned to media responsibilities.

Media Release - Part 2 - Media Outlets

I also understand that external media organizations may attend the Sherwood Park Stake, Moroni's Quest event. I give permission for my child's name, image, work and performance to be recorded (audio/video), copied, filmed or photographed for the purpose of being published and/or broadcast on-line, on television or radio.

I consent to the participant’s Works being published or broadcast by organizations external to the Sherwood Park Stake.
